Form Builder 


To Access the Form Builder, go Setup--> Database --> Form Builder.

To create a new form, click the green button +Create New Form.

Then the Create New Form page till open.


In the Create New Form page, provide a name for your form in the field labeled Form Name.

By default the form will be switched to being Active.  You can change the form to being inactive by switching the switch from blue to grey.


Next, name your Page 1 and add the first layers of fields, dropdowns and notes.  To keep your forms organized, you can add multiple pages.  Adding multiple pages with organized headers with related questions questions creates a multi-step form. Multi-step forms helps eliminate confusion and user intimidation.


When adding fields to be filled in, you can mark them Mandatory, easily move the order of the fields as needed, duplicate and remove fields and / or pages as needed.

In the Form Builder, you can also design your Form Theme with a color pallet, add pictures and texts.  You can also assign a form response email. The email in the Response Email field will be notified every time someone completes the form.





The Form Builder has the ability to receive payments. You can enable payments with the option to make payment mandatory, optional, setup recurring payments in the form and set pricing.

Once you have everything in place, you can Save and View your form.



After you Save, you can return to the Form List page to copy your form's URL to share on your website or in an email.


On the Form List you will also see you has responded and have the ability to View Responses and run export reports on these responses.



When you click View Responses, you will be redirected to the Form Response page where you can View the Details for an individual form response or for all form responses.  The responses can be exported as CSV and Excel.



When families submit their form, they will be prompted to confirm their form by adding their name and email.

Thus if you need to reach out to them, it is easy to do so and know exactly who filled out the form.
